Understanding Our Payment Policy: At our clinic, we prioritize providing comprehensive, personalized care. To achieve this, we operate on a fee-for-service basis and do not directly bill insurance companies.
Commercial Insurance: If you have commercial insurance (e.g., Blue Cross, Aetna, Cigna), you can submit your receipts for potential reimbursement. We'll provide you with the necessary documentation.
Medicare/Medicaid: Unfortunately, Medicare and Medicaid do not allow for reimbursement in this scenario.

Required Medical Information: To ensure we can provide the best possible care, please bring the following to your initial consultation:
Evidence of your pain (e.g., MRI reports, CT scans, NCV/EMG results, ultrasounds, lab tests).
Notes from your most recent appointment with your previous pain physician.
Notes from your most recent appointments with specialists (e.g., orthopedic surgeons, neurosurgeons).
Your prescription history for the past six months from your pharmacy.
Appointment Costs: Initial Consultation: $200
Comprehensive examination by a staff physician.
Review of your medical history and records.
Collaborative goal setting.
Development of a personalized pain management plan.
Assessment guidelines for ongoing care.
Detailed explanation of appropriate medications.
Referrals for additional screening, diagnostic testing (MRI, etc.), and non-pharmacological treatments (physical therapy, etc.), if needed.
Follow-Up Appointments (Continuum of Care): $150
Meetings with our physician and pain management professionals.
Evaluation of your personalized pain management plan.
Review of your goals and quality of life improvements.
Evaluation of new test results.
Continuation of your medication protocol.
Random Drug screening for compliance.
Required office visit every 21-28 days.
Referrals for additional testing or medication adjustments, if needed.
Important Policies:
Please bring all remaining medications prescribed by your previous pain center to your medication adjustment appointments. No exceptions.
We utilize the prescription drug monitoring program to ensure patient safety.
All patients undergo drug testing.
All patients are required to sign a pain management agreement.
