Required information
Please have the following patient information available when referring a patient:

• Diagnostic Study Confirming Chronic Condition (i.e. MRI, CT, Bone Scan, Lab, etc.)
• Last 3 visits from your office
• Recent notes from specialist concerning chronic problem if available
• Diagnosis
• Patient name & phone number
• Reason for referral

We will review the information and if accepted we will call the patient to schedule. We shall send you a fax confirming the appointment has been scheduled.
